In this document we outline the bereavement support needs of patients and their families as part of a continuum of care from illness into bereavement. We review current understandings of the grief and loss experience in the context of research, guidance and policy. This informs the evidence-based guidance for social workers in their support of patients and their families within specialist palliative care services.
